Overview
In Skillways you can add a survey as an activity. A survey can include short-answer or multiple-choice questions. You can randomize each. Unlike a quiz, you cannot assign each question a point or allow feedback for the questions. If you are grading a survey, you will need to assign it total points. A survey works well for an activity where you are not testing for knowledge but more for students’ opinions or thoughts.
It is important to note that any activity in Skillways must be attached to a page. You can add any content to the page. This is a great way to include an introduction to the activity or instructions. Alternatively, the page can be blank or only include a title if you prefer not to add any additional content. You can also add multiple activities to a page. For example you can have a survey and a file upload attached to the same page.
Add a Survey
To add a survey, go to the Edit tab from within the Learning Path.
Hover over the page or activity that you want the survey to come after and click on the three dots to the far right (1). Then click on new activity (2).
A new window will open that asks you what type of activity. Click on Survey.
A new window will appear.
Set Up Your Survey
Add the Survey name by double-clicking in the box at the top. Note: Make the Legend title short. It should just be a few words.
Select whether your survey should be active or remain in draft form. 
Check the box if you want to randomize the questions.
You can also enable conversation for the activity. Conversations are not graded. See the user guide How Do I Enable A Discussion Conversation In A Different Activity? on how to use this feature.

Select the type of Grading.
Note that surveys are graded on the total points and do not have the ability to assign each question a point. The grading can be “manual points” which means the learner would receive all points if they complete the survey or “participation” which allows for a complete/incomplete type of assignment without points associated. You can also choose to add a Rubric for grading. “None” means no grading is associated with the survey.

Select the Display settings
For the display (which will display to the students), check/uncheck if you want to show the number of questions that will be in the survey and the attempts
Add the Survey Questions
Click on add question under the Activity heading.
A new window will appear. Choose multiple choice or short answer. 
For multiple-choice
Add the question.
Then click on the choice button to add the response choices.
Add the response choice(s). 
At any time, you can delete the question or the choices by clicking on the red trash can.
For a short answer
Add the question.
When you are done, you can return to the Edit tab as everything saves automatically.
